These are available from 8.30 to 11.30am and from 3.30 until 6.00pm Monday
to Friday.
Appointments can be made in person or by telephone.
IF YOU CANNOT KEEP A BOOKED APPOINTMENT,
PLEASE TELEPHONE TO CANCEL SO THAT SOMEONE ELSE MAY TAKE ADVANTAGE OF
IT.
OPEN APPOINTMENTS
If you consider that your problem is urgent and cannot wait for an appointment,
we can see you as an extra but you may have to wait longer to be seen
as the doctor will also be seeing booked patients. You WILL NOT be able
to choose which doctor sees you. These open appointments are only available
between 8.30 and 10.30am and between 3.00 and 5.00pm. On Saturdays the
surgery is open between 8.30 and 11.30am for urgent cases only.

A SICKNESS CERTIFICATE ?
DO I NEED A SICK CERTIFICATE?
For employees, a sickness certificate completed and signed by the Doctor is only required for a period of continuous absence from work due to ill health for eight days or longer. For an absence of up to three working days no certificate is required. Between three and seven days' absence due to illness is covered by a self-certification form (SC1 or SC2) provided by your employer. These statutory DSS rules are universal and all employers are aware of them. If you require a sick note for insurance purposes etc, the Doctor will be able to issue a private certificate for which there is a charge (see current scale of charges displayed behind the reception counter). We only issue private certificates for the benefit of employers for under one week, at the discretion of the Doctor.

A HOME VISIT ?
Please
telephone before 10.00am if a visit is required. Please give the receptionist
as much information as possible to enable the doctor to allocate priority
to house calls.

DO I... OBTAIN
A REPEAT PRESCRIPTION?
These can only
be requested in writing, NOT by telephone/fax. Please write clearly your
full name, address, date of birth if you have a popular name and medication
required, including dose.
The prescription should be ready in 24 hours, but please take weekend
or bank holidays into account.
